Parts of a Disability Parking Form

The form itself is very simple. All there is to it are these structures:

  1. Identity of the disable person/representative
    First, the disable person will have to write some general information about them. This includes their full name, address, and contact number. Sometimes they have to give more detailed information like their ID card or driving license number.
  2. Questions about eligibility
