What is in a Performance Appraisal Form
To give an appraisal to someone, you need several structure to accommodate your feedbacks. Those structures are:
- Identity of reviewer
First, the reviewer or supervisor have to give some information about them. This includes their full name, job position, and/or ID card number.

- Information about appraisal

This is the core of the performance appraisal form. Here, the reviewer needs to see the questions being asked about the quality of the employee. Then, they should write down the ratings they would give to the employee.

The first and one of the most important feedback is to be objective. You need to throw away all your bias and feelings towards this person. The second tip is to think of your feedback. Don’t conclude that they have an aspect just because they show one action. You need to take your time to observe this person carefully. The next tip is to have a mentor. Doing things on our own is fine. But you will grow so much better if you have a mentor to guide you through the turmoils. Find someone that you look up to.
