A course evaluation form is a question naire which requires a written or typed response in order to answer to a series of questions in order to evaluate the instruction of a given course. The term may refer to a summary of responses to questionnaires.
The key purpose of this form is to produce feedback which the teacher and school can use to assess their quality of instruction.
The Process and Response of a Course Evaluation Form
The process of making a course evaluation form includes the following.
- Gathering information about the effect of learning and of teaching practice on student learning.
- Analyzing and interpreting the information gathered.
Responding to the results, is valuable for several reasons.
- They enable instructors to reflect on how others interpret their teaching methods.
The information can be also used by administrators, along with other input, to make summative decisions, such as decisions about promotion, tenure, salary and increases as well as make formative recommendations (identify areas where a course staff member needs to improve).
- They will create an overall picture of teaching performance.
Combined with peer evaluations, supervisor evaluations, and results of student’s test scores, the evaluation form will create an overall picture of teaching performance. Course evaluations implemented could be devided up to two ways, summative and formative.

Course evaluation form generally may include variety of skills and personality traits of teachers ranging from communication skills, organizational skills, enthusiasm, flexibility, attitude toward the student, teacher – student interaction, encouragement of the student, knowledge of the subject, fairness of grading and exams, and global student rating.

Summative evaluation occurs at the end of a semester, usually between week or two before the class ends. As its name, a summative evaluation will gather a summary of information regarding teaching process throughout the whole semester.
The evaluation is done by students of the class. Students have the option to reflect on an overall of instructors’ teaching without fear because course evaluations are completely confidential and commonly anonymous. This can be done either with a paper format or with online technology.
In a paper-based format, the paper form is distributed by students while the teacher is out of the class. Next, It is then sealed in an envelope and the teacher will not be able to see it until after final grades are submitted.
The online format can seem identical to a paper format. Both will allow the student to be able to provide feedback. This feedback will be used by teachers to reflect on the quality of their instruction. The information can also be used to examine the effectiveness of a teacher, particularly for promotion decisions.
Formative evaluation occurs when changes can take place during the current semester. This form of evaluation is performed by a peer consultation. Other experienced teachers will review their peer regarding on the instructions given. The purpose of this evaluation is for the instructor to consider receiving constructive criticism.
Generally, peer teachers will sit in on a few lessons given by the teacher and take notes for the evaluation. Later, the peer teachers will meet with the evaluated teacher and provide some informative feedback on their lessons. The peer team will offer suggestions on improvement, which the said teacher can implement.
Peer feedback is given in the form of an open session meeting. The peer teachers will first reflect on the qualities that were good. Then they move on to the areas that need improvements. Next the instructor will make suggestions for improvement and receive feedback on those ideas.
A Student feedback can be an important part in both summative and formative evaluation. Student evaluations could also be formative when their purpose is to help course staffs improve and enhance their teaching skills.
The teachers may require their students to participate in ongoing dialogue or directed discussions during the course of the semester. The use of a ‘Stop, Start, and Continue’ method for student feedback has been found to be highly effective at generating constructive feedback for course evaluation form.
