4 Important Structures

There is a general structure of a photo consent form that must be followed. Below are steps that can be followed to help you write a PCF the right way

1. Use Adobe PDF, Microsoft Word (.docx), or Open Document Text (.odt).

Releasors then have to include personal information that includes the full name, the address of the residence and the city and its state. 2. In this step, the release must also clearly expose the revocation clause.

Parental Photo Consent Form
